How do you shoot images like this one?

It´s really cool when the camera is this dark, and only a little bit of it is visible, but how do you do it? Is it possible to shoot images like this, or is it only Photoshopped? If so, how do you do that?

I'm guessing it's some sort of softbox that was used to get this effect.

I guess the flash above with - as Greg said - a softbox. Possibly combined by two or more shots.
